Momentum offers a range of options and locations across five counties in Southern California for people to find a “home” that is a match for their preferences and needs.
Our residential programming is structured to offer the highest level of personal care, supports, resources for people to enjoy a fulfilled and healthy lifestyle. Our services provide meaningful opportunities for continued learning, skill development, socialization, recreation, and personal advocacy. People living in any of our 22 group homes experience a loving, caring environment with support and programming tailored to each person’s individual needs. Our dedicated, highly trained staff members provide person-centered care designed to help residents live full, healthy, dignified lives.
- Our residential locations are designed for the comfortable accommodation of people who are either ambulatory or use a wheelchair for mobility
- Spacious single- and double-occupancy bedrooms are available and allow each individual to personalize
- Quality, healthy food choices are designed by trained dieticians
- Options for having access to nursing staff
- Residential settings and wrap-around services provide support programs and services that address the people’s individual needs
- Residences include free wireless internet access and free satellite television
- Residents receive in-home routine physician visits at least every 60 days
- Momentum as a fleet of wheelchair accessible vehicles to help residents have access to their community
