How does Momentum maintain privacy and HIPAA secure access to The Momentum Family Forum network and groups?
- Each person who utilizes Momentum services or programs and/or their guardians will be requested to complete a Momentum Photo Release Form to allow for photos/videos to be taken and published to The Momentum Family Forum and other Momentum-affiliated communications channels.
- Each family member seeking access to The Momentum Family Forum will be asked to complete a Family Access Profile Request Form to secure access and agree to the confidentiality and terms of the User Agreement.
- Access will be reviewed at each IPP meeting and can be revoked if secure access and confidentiality guidelines are not adhered to.
How will family communication work for this portal?
- Family members and guardians with Momentum Family Forum access will be able to comment directly on posts within the groups to which they have access.
- Additional questions may be sent to the Momentum Mighty Networks host at FamilyForum@momentum4all.org
- Expect to receive an inquiry response within 48 hours (M-F).
Are there any requirements to obtain access?
- Requesters must have a personal email address which will be utilized to receive information such as the login invite to The Momentum Family Forum.
- Requesters must be a current noted contact for a person using services at Momentum.
