Community Living Services is guided by the philosophy that people have the right, the desire, and the ability to make choices about their present and future lifestyles, and that they can and should play an active role in the decision-making that directly affects their lives.
Grounded in this philosophy, the approach to Community Living Services is supportive and flexible in nature. Adults are assisted, at the necessary level, to develop and achieve personal goals as the means to attaining personal growth and development. Outcomes are achieved that support the individual’s lifestyle choices. Services are provided in a manner that emphasizes individual strengths while simultaneously acknowledging and addressing medical, educational, physical, and social/emotional needs, empowering personal choice and fostering independence within truly integrated communities.
Momentum provides two types of services for individuals choosing to live in their own homes.
Independent Living Skills Training services are designed to assist individuals with acquiring and maintaining particular skills in areas of daily life.
Supported Living Services provide access to resources and support for individuals who require ongoing support in order to maintain their preferred lifestyle.
